对使用机械通气患者撤机过程中，每日早上采集患者的血样，进行B型尿钠肽（BNP）的监测，当BNP≥pg/mL时：限制液体摄入量（基线摄入量≤mL/24小时，肠外营养≤0mL/24小时，除了药物和营养物质之外无盐溶液摄入）；速尿使用的管理（即静脉注射剂量10~30mg/3小时，达到尿液排出量在4.5~9mL/kg/3小时的目的）。
摘要
背景
肺水肿可能会改变肺泡的细菌清除能力及易感性。因此通过减少液体负担的液体平衡处理可能影响插管患者VAP的发生。本次研究的目的是为了评估撤机过程中，液体管理策略对呼吸机相关并发症（VAC）和VAP的影响。
方法
本次研究数据来自欧洲和美国的9家ICU，一项关于撤机过程中B型尿钠肽（BNP）指导下的液体管理（BMW）的随机对照研究。为了使撤机过程标准化，所有患者使用基于计算机的自动撤机系统（AWS）。使用区组随机化的方法将患者分至干预组和对照组。干预组患者撤机过程中每日早上采集血样监测BNP，当BNP≥pg/mL，则限制液体摄入量（基线摄入量≤mL/24小时，肠外营养≤0mL/24小时，除了药物和营养物质之外无盐溶液摄入），速尿使用的管理（静脉注射剂量10~30mg/3小时，达到尿液排出量在4.5to9mL/kg/3小时的目的）。对照组在不知道BNP结果的情况下，按常规进行治疗包括速尿的使用。随访14天以上，使用竞争风险分析方法（FineandGray模型）比较两组间VAC和VAP的累积发病率。
结果
例患者中，41例发生VAP，常规护理组27例（17.8%），干预组14例（9.2%）（P=0.03）。将脱机结局作为一项竞争事件进行调整，比较两组间VAC和VAP的发病率明显下降[亚组危害比是0.44（0.22~0.87），P=0.02和0.50（0.25~0.96），P=0.03]。
结论
通过竞争风险分析发现，当机械通气患者开始撤机试验时，减少液体的液体管理策略可能会降低发生VAP的风险。
